Physical and Immunization Clinic

The clinic will be August 10th at the High School starting at 9:00.  Registration will close 8/3.  As of right now the clinic is from 9-11 but they will extend the clinic depending on need.  When the link is clicked, it takes you to a form to fill out an appointment request. There is a box for people to enter their preferred time for their appointment. Please keep in mind that this is a requested time, we will do our best to follow the requests.
