Immunization clinic

Macoupin County Public Health Department will hold an immunization clinic at Northwestern Elementary School on Tuesday, May 5th during school hours to give required vaccines for the upcoming 2015/2016 school year.  With your signed consent, your student can receive the required vaccine(s) and any others you request.  Consent forms must be returned to school by the end of the school day on Monday, May 4th to ensure enough vaccines are available.  Consent forms are available in the High School and Elementary Office.  Please contact the school nurse at 217-436-2442 if you have further questions.
